When I first heard about it there was an indication that they use a graduated scale in accordance with the age group. Even if they used a 4:1 scale, the epitestosterone levels are going to remain fairly static...they're not going to swing up right after an injection and drop as it's metabolized, so they'd keep the range that the ratio can fall into fairly tight.
I'm not 100% on how it's enacted, but the people I've talked to about it indicate that's what is supposed to happen. Henderson also has spoken about a consistent record of his own, privately-paid-for tests that could debunk the assumption that he's skyrocketing his levels between mandated tests.
