Fedor would be up there too, although sometimes it's hard to tell if he's just adapting as the fight goes, but he's definitely used some good game plans, like standing with Cro Cop and pushing the pace to throw him off. As for coaches, Greg Jackson seems to know to his stuff.

I would say Randy Couture.
Greg jackson was a very good choice too.
I would say no to Fedor. He just goes in and whoops ass.

Couture, and jacksons camps, i don't know but i would say Machida as well even if its just his style he tweeks it to adjust to his opponents tactics
