Big Nog is suffering from that currently. He was hard-ranked at one point. He was the #2 HW when he lost to Mir the first time, stayed in the top 5 by beating Couture, and then lost a couple of spots losing to Velasquez. He had a long lay-off and when he came back beat a top 10 in Schuab, lost to a higher ranked fighter in Mir, and then beat Ed Herman (unranked). If all that happened in a much shorter duration, his ranking wouldn't be as unstable. The problem is that many of those accomplishments are getting outdated. A bad to loss to Werdum hurts him badly. Barnett, in my opinion, is in the same boat.
Dave Herman, not Ed. Ed is a MW.

Imo, you guys need to merge 2 of your criteria: soft rankings & "based on where they were ranked at the time of the fight".
Let me explain. I do most of my activity on tapology.com now mostly because of the way they're setup to keep track of picks and individual rankings. NOBODY who picks all the fights picks above 65% so to simplify, 1 out of 3 picks are wrong. This needs to be taken into account when considering past fights. If the ranking was unjustified or "soft" then it should be adjusted accordingly depending on the outcome.
I think it's good that you're all willing to discuss your reasons for ranking. Discussion and an open mind can be amazing.
